All staff physicians assigned to Medicine Service will meet basic requirements and hold a degree of doctor of medicine or doctor of osteopathy from a college or university approved by the Administrator; complete an ACGME accredited residency satisfactory to the Administrator, meet the physical standards as described by the Administrator and will be licensed to practice without restrictions in a state or territory of the United States. Duties include but are not limited to the following: Provides guidance and supervision for the medical students and residents (interns, residents, and/or fellows) assigned to Medicine Service/subspecialty services and clinics according to the standards outlined in Center Policy Memorandum, 11-4, "Supervision of Postgraduate Residents," when assigned to do so by the Chief, Medicine Service. Participates in and attends medical center committees and/or teams as assigned by the Chief Medicine Service and in his/her absence. Participates in various facility programs designed to enhance and improve the delivery of patient care services, such as Continuous Quality Improvement (CQI) teams, as assigned by Chief, Medicine Service. Demonstrates commitment to high quality customer service, including specific measures identified as performance goals by the Medical Center. Adheres to all information security and privacy measures set by VHA and the Medical Center. Annually develops and evaluates professional and personal goals. Identifies individual professional learning needs and participates in formal programs, workshops, seminars, conferences, as well as self-directed learning activities, completes facility wide mandatory education yearly A physician, who is a supervisor, approves and disapproves leave for other staff physicians, technical and nursing personnel, and/or clerical personnel under his/her supervision. A physician, who is a supervisor, participates in the selection of new staff; prepares for promotions and recognition of outstanding performance of staff he/she supervises. A physician, who is a supervisor, revises work schedules and assignments on short notice to meet frequent, abrupt, and unpredictable changes in prescribed workloads and deadlines. Provides quality patient-centered patient care to both inpatients and outpatients for adults age 18-65 and older adults age 65+ as assigned by Chief, Medicine Service. Maintains clinical privileges in the areas of his/her expertise and in all areas required to carry out the duties of his/her assignments/responsibilities within Medicine Services. Demonstrate sound decision making skills, both clinical and administrative, which bring credit to the profession and serve to enhance the distinctive and contributory role of physicians. Facilitate the delivery of appropriate patient care in collaboration with other health professionals, medical center staff, and management. Serves as staff attending for sub-specialty consult service as assigned. Participates as a peer reviewer process when requested. Participates in multidisciplinary activities involving the treatment plan and discharge plans of patients admitted to the Medicine bed services. Systematically evaluates patient care programs in all Medicine venues. Exhibits leadership in initiating changes in patient care programs based on current concepts and finding from research and studies. Keeps informed of developments in the management and treatment of diseases and illness through continuing medical education, both formal and informal. Successfully integrates patient care activities on Medicine Services and other patient care programs at the medical center and in the community such as Social Work, Prosthetics, Home Based Health Care (HBHC), and private nursing homes. Consistently demonstrates expertise in diagnosing and treating seriously ill, multi-symptomatic patients when clinical assignments so dictate. Completes all medical record documentation in CPRS in a timely manner, including view alerts and encounter completion. Includes patient/family/significant other/health care provider in establishing realistic, measurable goals related to health education and treatment plan and in planning care on a continuum; assists staff in the process of mutual goal setting. Demonstrates skill in implementation of clinical practice guidelines and protocols for patients with complex medical care needs. Documents administrative and clinical information in a thorough and accurate manner. Utilizes problem solving methodologies to resolve problems that impact on patient care or staff performance. Plans implements, and evaluates care based on age specific requirements of the patient. Serves as a role model in assessing and evaluating patient status and identifying priority needs for care. Develops the initial plan of care and updates the plan, serving as a resource person to less experienced staff, and acts as a leader in developing standards of care. Makes decisions based on professional standards Assesses veterans by utilizing advanced clinical judgment and physical assessment skills. Identifies acute/non-acute/chronic health problems.
